It can be used in the event of an emergency
to allow emergency personnel to treat your minor children in your absence.
We will also put a copy of this form in our family binder to be used by babysitters.
It is important to fill this out right away and have it Notarized if required by your state.
Accidents happen, and it doesn't take a major event to find yourself in need of this form.
Lets hope we never need to use it.
