Vital records, Medical information

Today we are starting Section 2.  There are a few pages today to work on, DON'T feel overwhelmed.  You can make a special trip to get these records if you don't have them already, or if you have an appointment coming up, you can get them while you are there.
Make sure you put a copy of your Medical business cards in your business card section.
We will refer back to and build on some of these sections at a later time.
First, lets get all of our Dental records together.  Don't forget to include any x-rays, these can come in handy and can also be used in the unfortunate event they are needed for identification.

Next gather up all your Immunization and shot records.
Check to see if everyone is up to date and get any that are still needed.  I have two children needing shots for school.  One is going into Kindergarten and the other is going into 7th grade.
Don't forget to include your flu shot records.

Health Insurance is really important to keep track of.  I can't tell you how many times I have had to take a sick kid to the doctor and can't find my insurance card anywhere.  It is also important to have a copy of your policy and benefit packets.  I also like to include a list of Doctors my insurance will cover (including out of town Doctors and hospitals).  I put my cards in a sheet protector for trading cards.  You can also put business cards for Doctors and Hospitals in this area if you would like.

And LAST is a medical information page for each member of the family.  It is important to keep these pages UP TO DATE.
